OUTDOOR VISIT CONSENT

1) i agree to my child taking part in local visits and day trips,which may occur from time during the course of H4H programme.

2) I unterstand that the organizer will take all reasonable and proper precautions for the care and safety of my child.

3) I agree to inform the H4H Management of any relevant medical or other special circumtances affecting my child,including any treatment requried during the course of a visit.

4) I understand that if my child should need emergency medical treatment,every effort will be made to contact me before treatment is given. If, however this is impossible,I give my consent to my child undergoing emergency medical treatment.

1) I give permission to the management of Hope 4 Humanity to take photographs/videos of me and my child or children.

 2) I grant full right to the images resulting from the photography/video , and reproductions or adaptations of the images, publicity, or other purposes to help achieve the group's aim. This might include (but is not limited to),the right to use them in their printed and online publicity, social and print media.

please be aware that the children must be collected by named adults. Your children will not be allowed to go without the authorised adults.

Please follow the procedures naming two adults to collect your child. please note all children should pickup promptly.

we do not expect pupils or parents to change these arrangements. In an emergency, families will need to contact the organisers.
